Job Description:

Performs on-going casework activities to assist individuals in gaining access to medical, social, vocational, educational, and other needed services. Serves as primary case manager for caseload of clients who have co-occurring mental health and substance use disorder needs and will be responsible for service plan development; service linkage; coordination of clinic services; monitoring; advocacy; crisis resolution; skill building and liaison services. Ensuring all SAMSHA CCBHC expansion grant client NOMs data and documentation is entered into the SPARS system. Performs direct rehabilitative services that engage clients in activities designed to reduce the symptoms of and disabilities that result from Co-Occurring Psychiatric and Substance Abuse Disorder illness and to increase the ability of the individual to function in the community. Performs symptom management services to assist clients with the identification and management of the symptoms of substance use/mental illness, including monitoring or wellness checks and specific symptom management skills trainings. Performs psychosocial rehab and skills training services to assist clients with problem solving and to enable the client to maintain community living. Will provide education to clients and their families on mental health and substance use disorders and available interventions that may assist the client. Will assist in referrals when needed if the client needs more intensive SUD interventions. Duties of the position will require frequent interactions with individuals served, their families, and other service providers. Falsification of any document constitutes grounds for dismissal. Requires flexible hours, involves extensive paperwork, and will require self-directed performance. Travel and the use of a personal vehicle are required. Must have a valid Texas drivers license and a driving record that is acceptable for insurability through the Centers insurance carrier.

Bachelor's degree from an accredited college or university with a major in social, behavioral, or human service field required. Applicants must live within 70 mile radius of duty site.
