The Mental Health Case Manager will provide case management and
rehabilitative services to chronically mentally ill adults in a
community setting. Responsible for insuring quality of services
through planning, accurate documentation, coordination, direct
service provision, and quality review. Position demands working
closely with team members to provide services to all clients
assigned to ACT. Team members are responsible for continuity of
care for all clients and insuring all clients remain in community
setting. Assist individuals in obtaining transportation and/or
translation for the purpose of accessing services. Inform eligible
or potentially eligible individuals about services and how to
access them. Provide rehabilitative and case management
services.
Education Required: A Bachelor's degree from an accredited college
or university with a major in psychology, social work, medicine,
nursing, rehabilitation, counseling, sociology, human growth and
development, physician assistance, gerontology, special education,
educational psychology, early childhood education or early
childhood intervention or a bachelor's degree with at least 30
hours of coursework in the previous fields.
Experience Required: At least two years experience in mental health
field preferred.
Position Requirements: Valid Driver's License, proof of auto
insurance, and insurable through PermiaCare. Required to pass
criminal history and background checks as well as pre-employment
drug screen.
Full Time/Part Time: Full-time position
Salary: $21.76 per hour. Sign on bonus may be available.
