The Mental Health Case Manager will assist eligible individuals in
accessing medical, social, educational, and other appropriate
services that will help that individual achieve a quality of life
and community participation acceptable to the individual.
Components of service coordination include:
crisis prevention and management - locating and coordinating
services and supports to prevent or manage a crisis;
monitoring - ensuring that the individual receives needed services,
evaluating the effectiveness and adequacy of services, and
determining if identified outcomes are meeting the individuals
needs and desires as indicated by the individual;
assessment - identifying the nature of the presenting problem and
the service and support needs of the individual; and
service planning and coordination - identifying, arranging,
advocating, collaborating with other agencies, and linking for the
delivery of outcome-focused services and supports that address the
individual's needs and desires as indicated by the individual.
Education Required: A Bachelor's degree from an accredited college
or university with a major in psychology, social work, medicine,
nursing, rehabilitation, counseling, sociology, human growth and
development, physician assistance, gerontology, special education,
educational psychology, early childhood education or early
childhood intervention or a bachelor's degree with at least 30
hours of coursework in the previous fields.
Experience Required: At least one-year experience with individuals
with mental illness or developmental disabilities preferred.
Position Requirements: Valid Driver's License, proof of auto
insurance, and insurable through PermiaCare. Required to pass
criminal history and background checks as well as pre-employment
drug screen.
Full Time/Part Time: Full-time position
Salary: $20.65 per hour. Sign on bonus may be available.

Intellectual and Developmental Disabilities (IDD) Services,
Substance Use Disorder (SUD) Services, and Early Childhood
Intervention (ECI) Services.
