Job Description:
Encompass Health is seeking a dedicated LPN-LVN II to join our
team of healthcare professionals in providing compassionate,
high-quality care in a dynamic post-acute environment. As a valued
member of our team, you will work in a facility-based setting,
offering comprehensive services designed to aid in the recovery and
rehabilitation of our patients. This role is vital in ensuring the
well-being of patients through close collaboration with registered
nurses and physicians, managing care plans, monitoring patient
progress, and promoting optimal health outcomes. Administer
medications and injections to patients as prescribed by the care
plan. Monitor and record patient's condition and document the care
provided. Communicate with registered nurses, physicians, and
healthcare professionals to optimize patient outcomes. Assist with
patient mobility and physical therapy exercises. Educate patients
and families on health maintenance and disease prevention.
Collaborate with the healthcare team to assess, plan, implement,
and evaluate patient care plans. Proficient in administering
medications and managing IVs. Strong interpersonal and
communication skills. Ability to adhere to health and safety
guidelines reliably. Comprehensive knowledge of infection control
protocols. Skill in documenting and electronic health record (EHR)
systems. Medical Insurance Dental Insurance Vision Insurance 401k
Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse
(LVN) license. Basic Life Support (BLS) certification, preferably
from the American Heart Association.
