Job Description:
Encompass Health is actively seeking a dedicated LPN-LVN II to
join our compassionate team, providing superior care within our
integrated healthcare services system. This role involves working
across both facility-based and home-based settings to deliver
patient-centric care in line with our mission to offer
high-quality, cost-effective solutions across the post-acute
continuum. The ideal candidate will bring a passion for nursing and
a commitment to improving patient outcomes. You will work closely
with a multidisciplinary team to implement personalized care plans,
adhering to all regulatory and safety standards. Provide direct
nursing care according to the prescribed patient care plan. Monitor
patient health and record signs within patient records, maintaining
accuracy and confidentiality. Administer medications and treatments
as prescribed by the healthcare provider. Educate patients and
families on health care needs, conditions, and options. Ensure
compliance with all healthcare regulations and safety standards.
Report to registered nurses and doctors regarding patient health
concerns and conditions. Strong clinical skills in a healthcare
setting. Ability to operate medical equipment. Proficient in
nursing software and patient management systems. Excellent
communication and interpersonal skills. Detail-oriented with strong
observational skills. Medical Insurance Dental Insurance Vision
Insurance 401k Current LPN or LVN license in the state of
employment. CPR certified.
