documentation, and contribute positively to a team ensuring a safe,
healing journey for every patient. Administer nursing care to ill,
injured, or disabled patients within a post-acute care setting.
Monitor patient health and maintain accurate records of patient
therapy and treatment. Implement patient care plans in conjunction
with the healthcare team. Ensure the health and well-being of
patients, offering critical support for recovery and
rehabilitation. Educate patients and their families on health
conditions and post-acute care processes. Strong interpersonal and
communication skills. Ability to perform all technical aspects of
nursing care Proficient in maintaining accurate and complete
patient records. Capability to adhere to health and safety
guidelines faithfully and consistently. Medical Insurance Dental
Insurance Vision Insurance 401k Required: Current licensure as a
Licensed Practical Nurse/Licensed Vocational Nurse in the state of
practice. Preferred: BLS Certification.
