In this role, you will play a critical role in delivering personalized, high-quality care to our patients, focusing on a spectrum of care from the post-acute continuum to rehabilitation efforts. This position requires adaptability to both clinical settings and home care situations, demanding a versatile and compassionate approach to patient treatment. Administering nursing care to ill, injured, convalescent, or disabled patients Consulting and coordinating with healthcare team members to assess, plan, implement, or evaluate patient care plans Monitoring all aspects of patient care, including diet and physical activity Preparing patients for, and assisting with, examinations or treatments Recording patients' medical information and vital signs Modifying patient treatment plans as indicated by patients' responses and conditions Instructing patients on health management and post-treatment home care needs Administering medication and observing reactions Proficiency in maintaining accurate and detailed reports and records Adherence to all necessary nursing care standards Capability to operate and monitor medical equipment Strong communication and interpersonal skills Ability to handle emergency and crisis situations Medical Insurance Dental Insurance Vision Insurance 401k Current Licensed Practical/Vocational Nurse (LPN/LVN) license in the state of practice CPR certified
