paid for it!The CCBHC Care Coordinator will coordinate care across
a spectrum of health services, including access to high-quality
physical health and behavioral health care, as well as social
services, housing, education systems, and employment opportunities
as necessary to facilitate wellness and recovery of the whole
person. Care coordination closes gaps in treatment by helping
individuals access multiple types of services within the community
and provides follow-up to ensure the individual starts treatment
with those services to achieve their recovery goals. Care
coordinators will link individuals to integrated health care
programs, substance abuse programs, mental health services, veteran
services, and any other additional services needed. Care
coordinators will provide services under the Certified Community
Behavioral Health Clinic (CCBHC) model which offers a more
comprehensive, holistic, and integrated model of
treatment.Components of care coordination include:A.) Crisis
Prevention and Management - Locating and coordinating services and
supports to prevent or manage a crisis. Provide follow up crisis
care to assist individuals in locating and obtaining appropriate
professional help or self-help strategies within 7 days of release
from behavioral hospitalB.) Identifying Risks- - - -Care
coordinators will work with CCBHC manager, other staff members,
Utilization Management (UM) team, IT department, and UM committee
to analyze client specific data around risk factors and indicators
(i.e. evictions, crisis encounters, multiples behavioral
hospitalizations, etc.) which may flag that an individual has a
need for care coordination in order to access multiple services and
achieve recovery goalsC.) Holistic and Comprehensive
Care-identifying the nature of the presenting problem/need, closing
the gap between services provided by PermiaCare or other agencies,
providing appropriate professional referrals within PermiaCare or
to other agencies, and assisting the individual in accessing the
services identified through the referralsD.) Monitoring and
Linkage-monitor individual's treatment over several weeks to ensure
all needs have been addressed and continue to close gaps between
different services deliveries, complete follow-up assessments
determining if identified referrals and/or treatment are meeting
the individuals needs and recovery goals, assessing if additional
referrals are needed as indicated by the individualE.) Care
Coordination - identifying, arranging, advocating, collaborating
with other agencies for treatment options and services provided
that address client's needs individual. Expanding relationships
with other agencies through the development of memorandums of
understanding, sharing relevant health information, developing
referral networks, and utilizing community resources to their
fullest extent.Education Required: A Bachelor's degree from an
accredited college or university with a major in psychology, social
work, medicine, nursing, rehabilitation, counseling, sociology,
human growth and development, physician assistance, gerontology,
special education, educational psychology, early childhood
education or early childhood intervention or a bachelor's degree
with at least 30 hours of coursework in the previous
fields.Experience Required: One-year experience with individuals
with mental illness or developmental disabilities
preferred.Position Requirements: Valid Driver's License, proof of
auto insurance, and insurable through PermiaCare. Required to pass
criminal history and background checks as well as pre-employment
drug screen.Full Time/Part Time: Full-time positionSalary: $20.65
per hour. Sign on bonus may be available.PermiaCare is a public
