office, fieldPOSITION SUMMARY/JOB PURPOSE:The CSC (Coordinated
Specialty Care) Program Coordinator ensures that services are
provided with fidelity for the Coordinated Specialty Care First
Episode Psychosis (CSC FEP) program requirements. This program
serves individuals between 15 to 30 years old who are suffering
from their first onset of a psychotic disorder. The team works
weekly with individuals to assist them in employment or education
needs in addition to helping them understand their mental health
and work on recovery goals. The CSC Program Coordinator monitors
and tracks service delivery, maintains tracking documentation,
submits reports, and ensures clinical meetings are completed. This
position will provide direct supervision to the CSC Case Manager
while working closely with supportive employment specialists,
family partner specialists, peer support specialists, and medical
staff. The CSC Program Coordinator will provide direct services to
individuals in the program including skills training, case
management, psychoeducation, and counseling services.This position
works independently, under limited supervision, reporting major
activities through periodic meetings.EDUCATION, EXPERIENCE, OTHER
QUALIFICATIONS:Education Required: A Master's Degree in Counseling,
Psychology, Social Work, or a related subject area with full
licensure as an Licensed Professional Counselor (LPC), Licensed
Clinical Social Worker (LCSW), or Licensed Marriage and Family
Therapist (LMFT) is preferred. Professional licensure status of
LPC-Associate, LMFT-Associate, or LMSW under clinical supervision
is allowable for position.Experience Required: At least 1 year
experience in the social services field. At least 1 year of
supervisory experience preferred.Registration, Certification,
Licensure or other Qualifications Required:
- Must maintain a valid Texas Driver's license, auto liability
insurance and a driving record acceptable to PermiaCare's insurance
requirements.
- Required to pass criminal history and background checks as well
as pre-employment drug screen.
- Must obtain QMHP (Qualified Mental Health Professional)
certification within 6 months.ESSENTIAL DUTIES AND
RESPONSIBILITIES:
- Provide direct services to individuals in this population as
identified through recovery plan and TRR assessment.
- Provide direct supervision to CSC Case Manager.
- Monitor services delivered by Supportive Employment
Specialists, Family Partner, and Peer Support Specialists.
- Hold weekly CSC team meetings.
- Hold bi-weekly meetings with supportive employment
specialists.
- Track monthly performance numbers.
- Submit quarterly reports to Texas Health and Human Services
Commission (HHSC).
- Provide education and outreach about FEP to individuals in
services, family members, community, and local stakeholders.
- Link individuals in services to integrated services including
medical care, substance abuse treatment, crisis services, veteran
services, or any other needed services identified by clients.
- Assist CCBHC Manager with workflow and operations to ensure
services are meeting client needs and effective.
- Meet unit performance measures or targets.
- Maintain assigned caseload of individuals with mental
illness.
- Coordinate services to designated caseload.
- Enter accurate and appropriate documentation of services within
timeframe required.
- Maintain confidentiality of sensitive records and treatment
information, client files and protected health information in
compliance with HIPAA, laws, rules and regulations, and established
procedures.
- Maintain regular and reliable physical on-site attendance.
Regular attendance, dependability, and promptness are required for
the scheduled work day 100% of the time, to ensure consistency and
completeness of program's processes.
- Comply with the Abuse, Neglect, and Exploitation policy and
reporting requirements.
- Adhere to the Code of Conduct and Standards of Behavior policy
requirements.
- Establish and maintain effective work relationships with
individuals served and their families, supervisors, co-workers and
visitors by demonstrating cooperative, courteous and respectful
behavior at all times.
- Communicate regularly with supervisor.
- Open and process mail/email in a timely manner.
- Answer phone, collect phone messages and respond to requests
timely and accurately.
- Maintain safe and clean working environment by complying with
procedures, rules and regulations.

program in the community.

policies and procedures.
- Maintain valid and current driver's license, auto insurance,
acceptable driving record and reliable transportation at all times.
Driving may be required for this position.
- Complete all training as assigned prior to due date.

- Plan, organize, assign, supervise, review, and evaluate the
work of assigned employees.
- Participate in the hiring process and termination process of
assigned employees, including recommending selection of staff to
hire.
- Complete employee corrective actions as needed and in a timely
manner.
- Communicate regularly with assigned employees.
- Ensure compliance for performance evaluations for assigned
employees.
- Perform approval of timesheets on time.
- Develop, implement, and monitor new and revised operational
policies and work procedures.
- Monitor caseloads of assigned staff and assigns work
accordingly.
- Coordinate with Human Resources to respond to employee concerns
or complaints.
- Establish expectations and provide employee performance
feedback on an on-going basis, including ensuring understanding of
performance targets and goals.
- Provide orientation and on-going training, mentoring, and
coaching to assigned employees.
- Maintain quality and quantity of work performed within work
unit.
- Fill in for subordinates when needed to ensure coverage.
- Assist in resolving emergencies.
- Identify and resolve workplace problems.
- Ensure direct reports are aware of adhere to PermiaCare
policies and procedures.
- Ensure time is reported accurately.MARGINAL DUTIES AND

- Fill in for other MH staff as needed.
- Progress notes clearly show progress or lack of progress in
accordance with current provider agreement. (Meeting Minimum
Requirements of Current Internal Provider Agreement).
- Discharges and discharge summaries are completed when needed
and in accordance with regulatory guidelines.
- Identify client needs and client risks through administration
of TRR assessments and in collaboration with the CCBHC manager, the
UM team, the IT team, and the UM committee.
- Participate in community activities and/or attends community
meetings as needed.
