Job Description:
Touchstone Communities is seeking a dedicated and compassionate
Part-Time Licensed Social Worker to join our team, enhancing the
quality of life and overall well-being of our elderly residents.
This position requires an individual passionate about delivering
personalized, empathetic care within our social services
department. The successful candidate will be part of an
interdisciplinary team dedicated to proactive, resident-centered
care, focusing on social, emotional, and psychological needs.
Assess resident needs and develop individual care plans in
collaboration with other healthcare professionals. Provide
counseling and support to residents and their families, addressing
emotional and social needs. Facilitate and coordinate access to
community resources and services to improve quality of life.
Regularly evaluate interventions and adjust care plans as needed.
Document and maintain accurate records of social work activities
with residents. Participate in staff meetings and training sessions
to align with best practices in elderly care. Strong interpersonal
and communication skills. Ability to handle sensitive situations
with diplomacy and empathy. Knowledge of local healthcare and
social services networks. Proficiency in maintaining accurate and
detailed reports and records. Ability to work collaboratively in an
interdisciplinary team. 401k Current social work license in the
state of Texas. Certified Social Worker in Gerontology (CSW-G)
preferred.
