Job Description:

and skilled nurses who are dedicated to providing outstanding

health services, offering the necessary clinical expertise to help
patients recover more fully from illness or surgery within their
own homes. This position calls for nurses who are adept at
balancing technical nursing duties with the warmth and personalized
attention that every patient needs and deserves. Provide
high-quality patient care as prescribed in the patient’s care plan,
ranging from basic assessments to complex medical management.
Collaborate with physicians, therapists, and families to create and
implement comprehensive care plans. Educate patients and their
families on disease processes, treatment, and home health care
procedures. Monitor patient health condition, respond rapidly to
emergencies, and maintain detailed reports of patient care.
Administer medications and maintain accurate health documentation
and patient records. Guide and mentor junior nurses and nursing
assistants. Strong clinical skills with a robust understanding of
home health care protocols and best practices. Excellent
interpersonal and communication skills. Ability to work
autonomously and manage a health care plan effectively. Strong
organizational and time management skills. Medical Insurance Dental
Insurance Vision Insurance 401k Valid RN (Registered Nurse) license
required. CPR certified. Preferred: certification in Clinical Home
Health or Hospice and Palliative Nursing (CHPN).
